It is suggested that the cover and record card be printed on light purple cardboard; appendix 1 on yellow construction paper, appendix 2 on green construction paper and appendix 3 on blue construction paper. All other pages would normally be on white paper. However use whatever you have.
In some of the pages there is a request for an item to be added, look in the appendix pages for these items.
